Our providers participate in a number of medical insurance plans. Call your insurance provider to verify the doctor you wish to see is in your plan's network.

We do not accept any vision insurance plans as our providers are medical specialists.

HMO plans require a REFERRAL from your primary care physician (PCP) at the time of the appointment. It is the patient's responsibility to have a current insurance-authorized PCP referral for each visit with us. Please contact your PCP prior to your appointment to ensure the referral is processed in a timely manner. Patients without referrals will be rescheduled until a referral is obtained. Referrals can be faxed to our office at 815-363-8893.

Co-pays may be required for each office visit and are due at the time of service.

REFRACTIONS (glasses or contact fittings) are NOT a covered benefit of your MEDICAL insurance plan. If the doctor does a refraction, the patient is responsible for payment and it will be due at the time of visit. Please call our office for pricing of refraction exams.
